If you require any vaccinations relating to foreign travel you will need to complete the travel questionnaire below (paper copies are also available in reception) . One questionnaire is required for each member of the family who is registered at the Practice and is travelling.
Once the form has been submitted and reviewed, by the Practice Nurse, the practice will contact you to make the necessary arrangements.
The questionnaire includes questions on which countries and areas within countries that you are visiting to determine what vaccinations are required.
Please allow 12 weeks notice prior to your holiday. This will ensure you will be fully covered for any diseases.
